Albright College Athletics

The Sports Medicine Staff would like to welcome you to the Albright College Athletic Family. Prior to being eligible to compete in collegiate athletics it is required that all athletes new to Albright have: 

1. A onetime full physical exam performed by their family doctor prior to participation
2. Completed Health History Information Form
3. Completed Insurance Information Form
4. Completed informed consent form for random drug testing
5. Completed medical bills letter of responsibility 

*Failure to complete and return all forms will result in you being held from athletics participation.*
